Notice to Members of the Corporation

Re: 2013-2014 Membership Fees

Please note that at the June 20, 2012 Annual Genera Meeting the annual Membership fee was set at $2.00 and announced by the Board of Directors. The membership fee for existing members is due no later than April 30th, 2013 in order to maintain status as an active Member of the Corporation. 
New applicants must submit their application along with their membership fees no later than April 21, 2013 (60 days prior to the AGM) in order to be eligble to vote at the 2013 Annual General Meeting.

Members are asked to complete an updated Membership Form (V. 1.4) and return, along with their Membership fee, to 161 Applegrove Street, Sudbury, ON, P3C 1N2.

If you have not received a membership package by mail, and are interested in becoming a member/continuing to be a Member, please contact (705)-675-1596, ext. 256 with your current mailing address, or visit Reception at 161 Applegrove Street, in order to receive the package containing a 2013 SKHC Calendar, the fee notice, an updated Membership form and a self-addressed stamped envelope for easy return to the Head office of the Corporation.

Call for Nominations to the Board of Directors

The Shakagmik-Kwe Health Centre Board of Directors is currently accepting nominations to the Board for one (1) youth representative and one (1) community representative.

Please click here to see the instructions on submitting a nomination and the criteria set forth by the Board of Directors.
Nominations must be received by the Head Office of the Corporation no later than 4 p.m. on May 22nd, 2013.

Annual General Meeting

Please note that the Annual General Meeting will be held on June 20th, 2013 from 6pm – 8pm at the Steelworkers Hall located at 66 Brady Street, Sudbury, ON.
